On the morning of September 17, a date the World Health Organization has designated World Patient Safety Day, the safety measure on offer was a camera. And a microphone. And, in one of the more ambitious versions, a mirror that talks back to you while you brush your teeth.

The occasion was the closing lap of a European research network with a name that tells you the whole story if you let it: GoodBrother. Its chair, Dr. Francisco Florez-Revuelta of the University of Alicante, spent an interview marking the project’s outputs explaining how cameras, microphones, and machine vision can watch an aging body for the moment it falls, recognize the fall, and summon help faster than a human ever could. “Having a camera or microphone in your home can feel like surveillance,” he grants, before making the case that it does not have to. It is a genuinely appealing pitch. It is also the oldest pitch there is, the one that begins with your safety and ends with a device in the room that never stops listening.

The record on the program itself is not in dispute, because the program published it. COST Action CA19121 ran from September 2020 to September 2024, pulled in more than 48 countries and more than 200 researchers, and organized itself into five working groups spanning computing, engineering, healthcare, law, and sociology. It was funded through COST, the EU-supported body that finances transnational research networks, and it built, in its own description, a community of researchers and industrial partners. Four years, four dozen countries, a small standing army of academics and their industry counterparts, all convened around a single question: how to make audio and video monitoring welcome in the homes of older and frail people.

The name is not a curiosity. It is a confession, and the project put it in writing. Its own charter says GoodBrother will “offset the ‘Big Brother’ sense of continuous monitoring” by increasing user acceptance, exploiting these new solutions, and improving market reach. Read that last clause again. The stated deliverable is not fewer cameras or less listening. It is a population more comfortable with both, and a wider market to sell them.

The engineers, to their credit, took the surveillance problem seriously as an engineering problem. Florez-Revuelta notes that “some systems can focus on patterns of movement rather than detailed personal images,” a design choice that watches the shape of a person crossing a room without recording the person. The consortium produced its Trustworthy AAL Design Cards, a booklet titled “50 Questions on Active Assisted Living Technologies”, and a shelf of position papers on the ethical and legal tangles of pointing a lens at someone who lives alone. What they treated as solvable was the detection. What they left open was the consent, and then wrote into the charter that the fix for the open part was to make the objectors more accepting.

Because that is what “increasing user acceptance” is. When a public research program identifies the objection to its product, the objection being that a camera in the home is a camera in the home, and answers it by resolving to soften the objectors, it has told you which side of the transaction it works for. The concern GoodBrother itself names is “the possibility of collecting data, to whom it will be disseminated and how it will be used.” That is not a public-relations problem. It is the problem itself, and a design card does not dissolve it.


What four years did not produce is a single outcomes trial. There is nothing in the pile showing that homes wired with GoodBrother’s privacy-aware cameras logged fewer injuries, fewer hospitalizations, or one life saved against a control group that got a pendant alarm and a phone. The outputs are frameworks, questionnaires, position papers, and acceptance strategies. Meanwhile a 2026 qualitative study is still sitting older adults down to ask what they make of data privacy in smart technologies, which is not the posture of a field that considers the matter closed, and researchers elsewhere are asking in their own titles whether these are “Innovative Technologies or Invasive Technologies?” A separate line of work proposes the same class of monitoring as a way to address elder abuse in China, which is the tell. A device sold to protect a vulnerable person from harm is, by construction, positioned to see everything that happens to them, by whom, and when. The camera does not know it is supposed to only catch the fall.

Now that CA19121 has formally closed, its people have not scattered. Follow-on projects named in the same interview, MAYA and EMA_FRAILTY, carry the privacy-aware monitoring frameworks into new rooms: MAYA puts smart mirrors and AI conversational assistants in front of young cancer survivors, EMA_FRAILTY wires sensors to catch the early signs of frailty, and Dr. Gianluigi M. Riva, who chairs a separate COST action on science diplomacy, is a collaborator on the frailty work. The lens is moving from the hallway into the clinic. The apparatus that spent four years learning how to make continuous observation of the elderly feel acceptable is not winding down. It is finding new rooms.

Florez-Revuelta is not a villain, and the fall he wants to catch is real; anyone who has waited by a hospital bed knows the value of help that arrives in time. But a program that felt the need to answer the Big Brother comparison inside its own name has already conceded the argument. You do not spend four years and four dozen countries reassuring people about a thing they have no reason to fear.
